Equipment Breakdown - A breakdown of the equipment sold on lease. If the proposal is lengthy, you may want to attach the breakdown via an addendum to the lease, and fax that in with the application. (Mail hard copy of all forms)

Lease Rate Quoted - Will be finally determined by credit rating of account.
Miscellaneous - Supplier name and address, sales representative or contact, fax and phone numbers.
